  1. Provide compassionate and quality care to patients undergoing rehabilitation, ensuring their physical and emotional well-being.
  2. Assist patients with daily activities, including bathing, dressing, and grooming, in a respectful and dignified manner.
  3. Monitor and document patients' vital signs, intake and output, and assist with medication administration under the direction of a registered nurse.
  4. Collaborate with the healthcare team to develop and implement individualized care plans for patients, based on their specific needs and goals.
  5. Promote a safe and clean environment for patients by following infection control protocols and maintaining a clutter-free and organized workspace.
  6. Communicate effectively with patients, families, and other healthcare team members to provide updates on patients' progress and address any concerns or questions.
  7. Help patients improve their mobility and strength by assisting with therapeutic exercises and activities as directed by the physical or occupational therapist.
  8. Maintain accurate and timely documentation of patient care activities, including any changes in patients' condition or response to treatment.
  9. Participate in team meetings and contribute to the development of new ideas and strategies to improve patient care and outcomes.
  10. Demonstrate a commitment to ongoing learning and professional development by attending training sessions, workshops, and conferences related to healthcare and rehabilitation.

Job Qualifications
  • Certified Nursing Assistant (Cna) Certification: Uchealth Requires All Patient Care Assistants To Have A Valid Cna Certification. This Ensures That The Candidate Has Received Proper Training And Has The Necessary Skills To Provide Care To Patients.

  • Experience In Rehabilitation Care: Candidates Should Have Prior Experience Working In A Rehabilitation Setting, Such As A Hospital, Long-Term Care Facility, Or Outpatient Clinic. This Experience Will Demonstrate The Candidate's Understanding Of Rehab Protocols And Patient Needs.
