- Coordinate and manage the care of patients receiving organ transplants at UCHealth.
- Work closely with the multidisciplinary transplant team to ensure a smooth and efficient transplant process for patients.
- Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and understanding of care plans.
- Conduct thorough patient assessments, monitor and document patient progress, and make appropriate recommendations for treatment adjustments.
- Collaborate with other healthcare professionals to develop and implement individualized treatment plans for each patient.
- Educate patients and their families on the transplant process, potential risks and complications, and post-transplant care.
- Ensure adherence to transplant program policies and procedures to maintain regulatory compliance.
- Maintain accurate and complete patient records and documentation.
- Provide emotional support and counseling to patients and their families during the transplant process.
- Participate in transplant team meetings and case conferences to discuss patient progress and address any concerns.
- Stay up-to-date on current trends and advancements in transplant medicine through continuing education and professional development.

Active Rn License In The State Of Colorado: A Valid And Active Registered Nurse License In The State Of Colorado Is A Primary Qualification For A Transplant Coordinator At Uchealth. This Ensures That The Candidate Has Met All The Necessary Educational And Training Requirements To Practice As A Nurse In The State.
Bachelor Of Science In Nursing (Bsn) Degree: A Bsn Degree Is Preferred For The Transplant Coordinator Position At Uchealth. This Level Of Education Provides A Strong Foundation In Nursing Theory, Research, Leadership, And Critical Thinking Skills, Which Are Essential For The Role.
Minimum Of 2 Years Of Acute Care Experience: Candidates Must Have A Minimum Of 2 Years Of Experience As A Registered Nurse In An Acute Care Setting. This Experience Provides The Necessary Clinical Knowledge And Expertise To Manage Complex Transplant Patients.
Certification In Transplant Nursing: A Certification In Transplant Nursing, Such As Certified Clinical Transplant Coordinator (Cctc) Or Certified Transplant Nurse (Ctn), Is Highly Desirable For This Role. It Demonstrates A High Level Of Knowledge And Expertise In The Field Of Transplantation.
