  1. Conduct initial assessments of patients and their families to determine eligibility for hospice care.
  2. Explain the hospice program and services to patients and their families, providing reassurance and support during the admissions process.
  3. Collaborate with the interdisciplinary team to develop individualized care plans for each patient.
  4. Coordinate with healthcare facilities, physicians, and other referral sources to facilitate timely admissions.
  5. Ensure all necessary paperwork and documentation is completed accurately and in a timely manner.
  6. Maintain confidentiality and adhere to all HIPAA regulations.
  7. Assist patients and their families in understanding the hospice philosophy and goals of care.
  8. Provide emotional support and counseling to patients and their families during the admissions process.
  9. Evaluate patients' physical, emotional, and spiritual needs and provide appropriate referrals as needed.
  10. Educate patients and families on hospice services, policies, and procedures.
  11. Participate in on-call duties and emergency admissions as needed.
  12. Maintain accurate and up-to-date records of admissions and discharges.
  13. Collaborate with insurance companies and other payers to obtain authorization for hospice services.
  14. Participate in team meetings and communicate relevant information to the interdisciplinary team.
  15. Continuously strive to improve the quality of care provided to patients and their families.
  16. Adhere to all ethical and professional standards in accordance with VITAS policies and procedures.
  17. Attend and participate in training and development programs as required.
  18. Maintain a positive and professional demeanor at all times, representing VITAS in a positive light.
  19. Adhere to all safety and infection control policies and procedures.
  20. Other duties as assigned by the Admissions Manager or Director of Patient Care Services.

  • Current Rn Licensure In The State Of Practice
