Job Description

Attention all compassionate and dedicated nurses! VITAS Healthcare is seeking an experienced and driven Admissions Registered Nurse to join our dynamic team. As the first point of contact for patients and families, you will play a crucial role in providing exceptional end-of-life care.   1. Conduct initial patient assessments and determine eligibility for hospice care.
  2. Communicate effectively with patients, families, and interdisciplinary team members.
  3. Provide emotional support and education to patients and families regarding end-of-life care.
  4. Collaborate with physicians to develop and implement individualized care plans.
  5. Administer medications and treatments as prescribed by the physician.
  6. Monitor and report changes in patient's condition and collaborate with the interdisciplinary team to adjust care plans accordingly.
  7. Document all patient care activities accurately and in a timely manner.
  8. Provide support and guidance to families and caregivers, including education on symptom management and end-of-life care.
  9. Serve as a liaison between patients, families, and VITAS Healthcare to ensure continuity of care.
  10. Adhere to all established policies, procedures, and protocols in accordance with state and federal regulations.
  11. Participate in ongoing education and training to maintain and enhance clinical skills.
  12. Display empathy, compassion, and professionalism in all interactions with patients, families, and team members.
  13. Serve as a positive and proactive member of the interdisciplinary team, promoting a collaborative and supportive work environment.
  14. Advocate for patients' rights and ensure they receive high-quality, dignified care.
  15. Continuously strive to improve the quality of care provided to patients and their families.

Job Qualifications
  • Bachelor's Degree In Nursing From An Accredited Institution.

  • Valid Rn License In The State Of Employment.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In Hospice Or Home Health Care.

  • Strong Assessment And Critical Thinking Skills.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Interdisciplinary Teams And Effectively Communicate With Patients And Families.
