Job Description

Do you have a strong background in healthcare and a desire to make a difference in the lives of others? If so, VITAS Healthcare is seeking a dedicated Health System Navigator to join our team. As a Health System Navigator, you will serve as a liaison between patients, families, and healthcare providers, ensuring that our patients receive the best possible care. We are looking for individuals with excellent communication skills, a deep understanding of healthcare systems, and a compassionate and empathetic approach. Join us in our mission to provide quality end-of-life care and support to our patients and their families.

  1. Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.
  2. Utilize strong communication skills to educate patients and families on healthcare options and navigate them through the complexities of the healthcare system.
  3. Build and maintain relationships with healthcare providers to ensure continuity of care for patients.
  4. Advocate for patients' needs and preferences to healthcare providers to ensure personalized and compassionate care.
  5. Stay updated on current healthcare policies and regulations to ensure compliance and provide accurate information to patients and families.
  6. Collaborate with interdisciplinary team members to develop individualized care plans for patients.
  7. Conduct thorough assessments of patients' medical, social, and emotional needs to identify potential barriers to care.
  8. Work closely with insurance companies and healthcare facilities to coordinate services and obtain necessary authorizations.
  9. Support patients and families in making difficult healthcare decisions by providing resources and guidance.
  10. Document all interactions and updates in patients' medical records to ensure accurate and timely communication among healthcare providers.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Social Work, Or Related Field.

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In A Patient Navigation Or Care Coordination Role.

  • Strong Understanding Of Healthcare Systems, Including Insurance Coverage And Reimbursement Processes.

  • Excellent Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Patients, Families, And Healthcare Providers.

  • Proficient In Using Electronic Medical Records And Other Healthcare Software Systems.
