- Provide emotional support and counseling to patients and their families during difficult times.

- Collaborate with other members of the healthcare team to develop and implement individualized care plans for patients.
- Advocate for patients and assist them in accessing necessary resources and support services.
- Educate patients and their families about their illness, treatment options, and available resources.
- Communicate and coordinate with community organizations and agencies to facilitate referrals and coordinate care for patients.
- Maintain accurate and thorough documentation of patient interactions and progress.
- Participate in interdisciplinary team meetings to discuss patient care and provide input on psychosocial issues.
- Provide crisis intervention and support during end-of-life care.
- Adhere to ethical and legal standards of practice and maintain confidentiality of patient information.
- Attend trainings and continuing education opportunities to stay current on best practices and regulations related to social work and end-of-life care.
- Collaborate with hospice chaplains and bereavement counselors to provide holistic support to patients and their families.
- Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.
- Maintain positive relationships with patients, families, and colleagues, fostering a supportive and compassionate work environment.
- Adhere to VITAS Healthcare's mission, values, and policies.
Bachelor's Or Master's Degree In Social Work: This Is A Foundational Requirement For Any Social Worker Position, And It Demonstrates A Strong Understanding Of Social Work Principles And Practices.
State Licensure And/Or Certification: Vitas Healthcare May Require Social Workers To Hold A Valid State License Or Certification, Such As Licensed Clinical Social Worker (Lcsw) Or Certified Social Worker (Csw).
Experience Working In A Healthcare Setting: Vitas Healthcare Is A Hospice And Palliative Care Provider, So Experience Working In A Healthcare Setting Is Highly Valuable For A Social Worker In This Role. This Could Include Previous Employment In A Hospital, Nursing Home, Or Home Health Agency.
Excellent Communication And Interpersonal Skills: Social Workers At Vitas Healthcare Must Be Able To Effectively Communicate With Patients, Families, And Interdisciplinary Team Members. Strong Interpersonal Skills Are Also Necessary For Building Relationships And Providing Support To Patients And Their Loved Ones.
Knowledge Of End-Of-Life Issues And Bereavement Support: As A Hospice And Palliative Care Provider, Vitas Healthcare Requires Social Workers To Have A Deep Understanding Of End-Of-Life Issues And The Ability To Provide Compassionate Support To Patients And Their Families During This Difficult Time. Experience In Bereavement Counseling Is Also A Plus.
