  1. Provide compassionate and respectful care to patients in their final stages of life.
  2. Assist patients with personal care tasks such as bathing, dressing, and grooming.
  3. Administer medications as prescribed by the hospice team.
  4. Monitor and report changes in patients' physical and emotional conditions.
  5. Support patients' families by providing emotional support and maintaining open communication.
  6. Collaborate with the hospice team to develop and implement individualized care plans.
  7. Ensure patients' safety and comfort at all times.
  8. Maintain accurate and timely documentation of patient care.
  9. Adhere to all hospice policies and procedures.
  10. Attend and actively participate in team meetings and trainings.
  11. Continuously strive for professional growth and development.
  12. Uphold the mission and values of Yale New Haven Health System.
  13. Exhibit empathy and understanding towards patients and their families during their most vulnerable moments.
  14. Maintain confidentiality and respect the privacy of patients and their families.

Job Qualifications
  • Certified Nursing Assistant (Cna) Or Home Health Aide (Hha) Certification From An Accredited Program

  • Minimum Of 1 Year Of Experience In A Hospice Or Home Health Setting

  • Knowledge Of Hospice Care Principles And Techniques

  • Ability To Provide Compassionate And Empathetic Care To Patients And Their Families

  • Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team.
