Social Worker

Yale New Haven Health System is seeking a compassionate, dedicated Social Worker to join our team in providing high-quality care to our patients and families. As a Social Worker, you will be responsible for assessing and intervening in the social, mental health, and emotional needs of our patients. With your excellent communication and empathy skills, you will also provide support to families and help connect them to resources in their community. Our ideal candidate is someone who is passionate about providing health care with a holistic, patient-centered approach. To be successful in this role, you will need to have a Master's degree in Social Work and be licensed in the state of Connecticut. In addition, you should have a minimum of three years of experience in a health care setting. Knowledge of medical terminology, psychosocial and mental health assessment, and discharge planning is also essential. A commitment to cultural competency and excellent customer service is a must.
