  1. Provide compassionate and empathetic support to cancer patients and their families throughout their treatment journey.
  2. Conduct thorough assessments of patients' psychosocial needs and develop individualized care plans.
  3. Collaborate with interdisciplinary team members to ensure comprehensive and coordinated care for patients.
  4. Offer emotional support and counseling to patients and their loved ones, addressing their concerns and helping them cope with the challenges of a cancer diagnosis.
  5. Connect patients and families with appropriate resources and support services, both within and outside the healthcare system.
  6. Advocate for patients and their families, ensuring their needs are met and their voices are heard.
  7. Assist patients with navigating the healthcare system, including insurance coverage and financial assistance.
  8. Maintain accurate and thorough documentation of patient interactions and progress.
  9. Stay up-to-date on current oncology treatments and best practices in oncology social work.
  10. Participate in team meetings and trainings to enhance knowledge and skills related to oncology social work.
  11. Adhere to all ethical and professional standards in accordance with the National Association of Social Workers (NASW) Code of Ethics.
  12. Collaborate with community organizations and partners to provide additional support and resources for patients.
  13. Continuously evaluate and improve the effectiveness of the social work program within the oncology department.
  14. Maintain confidentiality and privacy of all patient information.
  15. Serve as a positive and professional representative of Yale New Haven Health System.

Job Qualifications
  • Master's Degree In Social Work: A Candidate For This Position Should Hold A Master's Degree In Social Work From An Accredited Institution. This Degree Provides The Necessary Knowledge And Skills To Work With Individuals And Families Affected By Cancer.

  • Licensure: A Valid License To Practice Social Work In The State Of Connecticut Is Required For This Position. This Ensures That The Candidate Has Met The Necessary Qualifications And Standards To Provide Social Work Services.

  • Experience In Oncology: The Ideal Candidate Should Have Experience Working In An Oncology Setting, Preferably In A Healthcare System. This Experience Will Provide A Strong Understanding Of The Unique Needs And Challenges Faced By Cancer Patients And Their Families.

  • Strong Communication And Interpersonal Skills: A Social Worker In This Role Will Be Working Closely With Patients, Families, And A Multidisciplinary Healthcare Team. Therefore, Strong Communication And Interpersonal Skills Are Essential To Effectively Support And Advocate For Patients.

  • Understanding Of Psychosocial Issues: Cancer Not Only Impacts A Person's Physical Health But Also Their Emotional, Social, And Financial Well-Being. A Social Worker In This Role Should Have A Strong Understanding Of Psychosocial Issues Related To Cancer And The Ability To Provide Appropriate Support And Resources To Patients And Families.
