  1. Provide emotional and psychological support to patients and their families during end-of-life care.
  2. Assist patients and families in navigating the challenges of terminal illness.
  3. Conduct comprehensive assessments of patients' needs and develop individualized care plans.

  11. Stay up-to-date on current trends and research in the field of end-of-life care.

  14. Collaborate with community resources and organizations to provide additional support to patients and families.
  15. Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.

Job Qualifications
  • Master's Degree In Social Work: A Candidate For This Position Must Possess A Master's Degree In Social Work From An Accredited University Or College.

  • Certification In Palliative Care: The Ideal Candidate Should Have A Certification In Palliative Care, Demonstrating Their Knowledge And Expertise In This Specialized Field.

  • Experience In Hospice Or Palliative Care: Previous Experience Working In Hospice Or Palliative Care Settings, Preferably In A Healthcare Setting, Is Highly Desirable For This Role.

  • Strong Communication And Counseling Skills: A Social Worker In Palliative Care Must Possess Excellent Communication And Counseling Skills To Effectively Support Patients And Their Families During This Difficult Time.

  • Knowledge Of Healthcare Systems And Resources: A Candidate For This Position Should Have A Thorough Understanding Of Healthcare Systems And Resources, Including Insurance Coverage, Medical Equipment, And Community Support Services, To Assist Patients In Navigating Their Care.
