  1. Provide emotional support and counseling to patients and their families during end-of-life experiences.
  2. Collaborate with interdisciplinary team members to ensure comprehensive and coordinated care for patients.
  3. Conduct assessments to identify the psychosocial needs of patients and develop individualized care plans.
  4. Offer education and resources to patients and families about palliative care and end-of-life decisions.
  5. Advocate for patients' rights and assist in navigating complex healthcare systems.
  6. Facilitate communication and decision-making between patients, families, and healthcare providers.
  7. Document all interactions and interventions in a timely and accurate manner.
  8. Participate in interdisciplinary meetings to review patient progress and make recommendations for care.
  9. Stay up-to-date on current research and best practices in palliative care.
  10. Maintain confidentiality and adhere to ethical standards in all interactions with patients and families.
  11. Provide crisis intervention and support during difficult situations.
  12. Collaborate with community organizations and resources to provide additional support to patients and families.
  13. Communicate effectively with patients and families from diverse backgrounds.
  14. Adhere to all policies and procedures of the Yale New Haven Health System.
  15. Attend trainings and continuing education opportunities to enhance knowledge and skills in palliative care.

Job Qualifications
  • Master's Degree In Social Work: Candidates Must Possess A Master's Degree In Social Work From An Accredited Institution. This Ensures That They Have The Necessary Knowledge And Skills To Provide Effective Palliative Care To Patients And Their Families.

  • Experience In Palliative Care: Candidates Must Have A Minimum Of 2 Years Of Experience Working In A Palliative Care Setting. This Experience Will Demonstrate Their Ability To Provide Emotional Support And Counseling To Patients With Life-Limiting Illnesses.

  • Knowledge Of End-Of-Life Care: Candidates Should Have A Strong Understanding Of End-Of-Life Care Principles And Practices. This Includes Knowledge Of Pain Management, Advance Care Planning, And Grief Counseling.

  • Excellent Communication Skills: Social Workers In Palliative Care Must Have Excellent Communication Skills To Effectively Collaborate With Patients, Families, And Interdisciplinary Teams. They Must Also Be Able To Communicate Complex Medical Information In A Compassionate And Understandable Manner.

  • Licensure And Certification: Candidates Must Hold A Valid Clinical Social Work License In The State Of Connecticut. They Should Also Have Certification In Palliative Care From A Reputable Organization Such As The National Association Of Social Workers (Nasw). This Ensures That They Adhere To Ethical Standards And Are Up-To-Date With The Latest Practices In Palliative Care.
