As an Anticoagulation Clinic Pharmacist, you will play a crucial role in providing specialized pharmaceutical care to patients who are on anticoagulation therapy.   1. Provide pharmaceutical care to patients on anticoagulation therapy within the Anticoagulation Clinic.
  2. Ensure safe and appropriate medication use for patients by conducting medication therapy reviews, monitoring drug interactions, and reviewing patient medical histories.
  3. Collaborate with physicians and other healthcare professionals to develop and implement individualized treatment plans for patients.
  4. Monitor and adjust medication dosages based on patient response and laboratory results.
  5. Educate patients on the proper use of anticoagulation medications, potential side effects, and the importance of adherence to treatment.
  6. Conduct regular follow-up appointments with patients to assess their progress and make any necessary changes to their treatment plan.
  7. Maintain accurate and up-to-date patient records and documentation.
  8. Stay informed about new developments and advancements in the field of anticoagulation therapy and incorporate them into practice.
  9. Participate in quality improvement initiatives and assist in the development of policies and procedures related to anticoagulation therapy.
  10. Provide guidance and support to pharmacy technicians and other staff members as needed.
  11. Adhere to all state and federal regulations and maintain compliance with all pharmacy laws and standards.
  12. Demonstrate a commitment to patient-centered care and maintain a high level of professionalism and compassion in all interactions with patients and colleagues.
  13. Continuously strive to improve patient outcomes and contribute to the overall success of the Anticoagulation Clinic and Baptist Health.

Job Qualifications
  • Pharmd Degree And Active Licensure In The State Of Kentucky

  • Minimum Of 2 Years Of Experience In An Anticoagulation Clinic Setting

  • Strong Knowledge Of Anticoagulant Medications And Their Management, Including Monitoring And Dosing Protocols

  • Excellent Communication And Patient Counseling Skills

  • Ability To Work Collaboratively With A Multidisciplinary Team And Maintain Accurate Patient Records In An Electronic Health Record System.
