Job Description

Baptist Health (Kentucky) is seeking a compassionate, detail-oriented Case Management Social Worker to join our team. In this role, you will have the opportunity to make a difference in the lives of individuals and families by providing case management services. You will work collaboratively with a variety of other healthcare professionals to identify individual needs and goals, and develop and implement plans for achieving those goals.To be successful in this role, you must possess a Master’s degree in Social Work and a valid social work license in the state of Kentucky. You must also have a minimum of two years of experience providing case management services in a healthcare setting. An understanding of the healthcare system and the ability to think critically and solve problems are essential. You must also possess strong interpersonal skills, be organized and be able to work independently.If this opportunity excites you, we would love to hear from you.

Responsibilities:

  1. Provide case management services to individuals and families to identify individual needs and goals.
  2. Develop and implement plans to achieve individual goals.
  3. Collaborate with other healthcare professionals to ensure optimal care.
  4. Utilize critical thinking and problem solving to navigate the healthcare system.
  5. Maintain organized records and documents of all case management services.
  6. Remain up-to-date on all relevant laws and regulations in the state of Kentucky.
  7. Communicate effectively with patients, families, and other healthcare professionals.
  8. Demonstrate compassion and empathy when interacting with patients and families.
