Case Management Social Worker

Baptist Health (Kentucky) is seeking a compassionate, detail-oriented Case Management Social Worker to join our team. In this role, you will have the opportunity to make a difference in the lives of individuals and families by providing case management services. You will work collaboratively with a variety of other healthcare professionals to identify individual needs and goals, and develop and implement plans for achieving those goals.To be successful in this role, you must possess a Master’s degree in Social Work and a valid social work license in the state of Kentucky. You must also have a minimum of two years of experience providing case management services in a healthcare setting. An understanding of the healthcare system and the ability to think critically and solve problems are essential.
