- Develop and implement clinical pharmacy programs and services to improve patient outcomes and enhance the quality of patient care.
- Conduct medication therapy reviews and provide recommendations for appropriate medication use, dosing, and monitoring.
- Collaborate with healthcare teams to ensure safe and effective medication use and promote interprofessional communication.
- Provide drug information and education to healthcare professionals, patients, and caregivers.
- Monitor and manage medication therapy for high-risk patients, including those with complex medical conditions or multiple medications.
- Participate in quality improvement initiatives and contribute to the development of evidence-based medication protocols and guidelines.
- Conduct medication reconciliation and identify and resolve medication-related discrepancies.
- Stay current with new drugs, therapies, and guidelines, and incorporate them into practice as appropriate.
- Serve as a resource for pharmacy staff and provide guidance and support for medication-related issues.
- Maintain accurate and complete patient records and documentation of clinical interventions and outcomes.
- Comply with all regulatory requirements, including state and federal laws, and maintain a high level of ethical standards.
- Participate in continuing education and professional development activities to enhance knowledge and skills in clinical pharmacy practice.
- Foster a culture of patient safety and medication error prevention through the implementation of best practices and quality improvement initiatives.
- Act as a preceptor for pharmacy students and residents and provide guidance and mentorship to new pharmacists.
- Actively contribute to the success and growth of the pharmacy department and organization as a whole.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: A Clinical Pharmacist Specialist At Baptist Health Must Possess A Pharmd Degree To Provide Advanced Clinical Knowledge And Expertise In Pharmaceutical Care.
State Licensure: The Candidate Must Be Licensed To Practice Pharmacy In The State Of Kentucky, Or Eligible To Obtain A License Upon Hire.
Board Certification In A Specialized Area Of Pharmacy: Baptist Health Prefers Candidates Who Are Board Certified In A Specialized Area Of Pharmacy Such As Ambulatory Care, Critical Care, Oncology, Or Pharmacotherapy.
Minimum Of 2 Years Of Experience In A Clinical Pharmacy Role: The Ideal Candidate Should Have At Least 2 Years Of Experience As A Clinical Pharmacist, Preferably In A Hospital Or Healthcare Setting.
Strong Communication And Interpersonal Skills: As A Clinical Pharmacist Specialist, The Candidate Will Work Closely With Physicians, Nurses, And Other Healthcare Professionals. Therefore, Excellent Communication And Interpersonal Skills Are Essential For This Role.
