  1. Provide pharmaceutical care to patients by conducting medication therapy reviews, drug utilization evaluations, and drug therapy interventions.
  2. Collaborate with healthcare providers to develop and implement treatment plans for patients.
  3. Stay up-to-date on the latest pharmaceutical advancements and make recommendations for their implementation in patient care.
  4. Utilize clinical knowledge and expertise to ensure safe and effective medication use for patients.
  5. Monitor medication therapy outcomes and make adjustments as needed.
  6. Serve as a resource to healthcare providers and patients on medication-related issues.
  7. Participate in interdisciplinary teams to improve patient outcomes and quality of care.
  8. Maintain accurate and up-to-date patient records and documentation.
  9. Adhere to all state and federal regulations and guidelines related to pharmaceutical care.
  10. Provide education and training to other healthcare professionals on medication-related topics.
  11. Participate in research and quality improvement initiatives.
  12. Maintain a high level of professionalism and ethical standards in all interactions with patients and colleagues.
  13. Act as a leader and mentor to other pharmacists and pharmacy staff.
  14. Participate in the development and implementation of medication-related policies and protocols.
  15. Continuously strive to improve patient care and outcomes through ongoing professional development and education.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Program.

  • Licensure As A Registered Pharmacist In The State Of Kentucky.

  • Completion Of A Postgraduate Clinical Pharmacy Residency Program.

  • Board Certification In A Clinical Specialty Such As Ambulatory Care, Critical Care, Or Oncology.

  • Minimum Of 3-5 Years Of Experience As A Clinical Pharmacist In A Hospital Or Healthcare System Setting.
