  1. Perform accurate and timely medication reconciliation for patients admitted to the hospital, including obtaining and verifying medication histories from patients, caregivers, and other healthcare providers.
  2. Collaborate with physicians, nurses, and other healthcare professionals to ensure accurate and safe medication regimens for patients.
  3. Identify and resolve discrepancies or potential drug-related problems in medication histories.
  4. Utilize electronic health record systems and other technology to document and track medication reconciliation processes.
  5. Educate patients and caregivers on medication regimens and provide counseling on proper medication use.
  6. Monitor and report medication errors and adverse drug reactions to healthcare team and appropriate regulatory agencies.
  7. Adhere to all hospital policies and procedures related to medication reconciliation and patient safety.
  8. Maintain a clean and organized work environment, including proper handling and storage of medications.
  9. Participate in continuous education and training to stay current on medication reconciliation best practices and healthcare regulations.
  10. Foster a positive and collaborative work environment by communicating effectively and working as a team player with other healthcare professionals.
  11. Follow all legal and ethical guidelines regarding patient confidentiality and rights.
  12. Demonstrate a strong attention to detail and accuracy in all aspects of medication reconciliation.
  13. Prioritize tasks and manage time effectively to meet the demands of a fast-paced healthcare environment.

Job Qualifications
  • Certified Pharmacy Technician (Cpht) License In The State Of Kentucky

  • Minimum Of 1-2 Years Experience In A Pharmacy Setting, Preferably With Medication Reconciliation Experience

  • Strong Knowledge Of Medication Names, Dosages, And Drug Interactions

  • Excellent Attention To Detail And Organizational Skills
