Social Worker

At CHI St. Alexius Health, we are seeking a passionate and experienced Social Worker to join our team. Our Social Workers use knowledge of human behavior and social systems to help individuals, families, and groups enhance their social, emotional, and economic well-being. As a Social Worker, you will provide clinical assessment, counseling, and case management services to individuals in need. You will guide patients and their families in dealing with a variety of issues, including mental health, relationship problems, substance abuse, abuse, and health-related concerns.To be successful in this role, we are looking for a candidate with a Master's degree in Social Work, and at least one year of experience working with individuals, families, and groups. In addition, you should have strong communication skills, excellent problem-solving and decision-making abilities, and an empathetic and nonjudgmental attitude. This position requires flexibility, as you may be asked to work nights and weekends. Responsibilities:

1. Provide clinical assessment, counseling, and case management services to individuals in need.

3. Maintain confidentiality of patient information and adhere to the ethical standards of the profession.
4. Develop individualized treatment plans to address the needs of each patient.

6. Educate patients and their families on available resources and services.
7. Maintain accurate records of patient care.
8. Work collaboratively with other healthcare professionals to ensure the best care for patients.
9. Remain current with the latest developments in social work and mental health.
10. Flexibility to work nights and weekends.
