Social Worker

CHI St. Alexius Health is looking for a compassionate and highly motivated Social Worker to join our team. In this role, you will use your expertise to help patients and families cope with social, emotional, and financial issues that can arise during their time in the hospital. You will provide therapeutic interventions, counseling, and support to patients and families in order to promote their emotional and physical well-being.We are looking for an individual who holds a MSW or similar degree and at least two years of professional social work experience. The ideal candidate will be a team player with excellent interpersonal skills, a strong work ethic, the ability to work in a fast-paced environment, and a commitment to providing exemplary patient care. Responsibilities:

1. Provide therapeutic interventions, counseling, and support to patients and families in order to promote their emotional and physical well-being.
2. Assist patients and families in coping with social, emotional, and financial issues that can arise during their time at the hospital.
3. Maintain accurate and up-to-date patient records in accordance with HIPAA and other applicable regulations.
4. Develop and implement individualized treatment plans that are tailored to the needs of each patient.
5. Collaborate with other departments and healthcare providers to ensure that patients receive the best possible care.
6. Monitor patient progress and update treatment plans as needed.
7. Provide crisis intervention services as needed.
8. Participate in interdisciplinary team meetings and other activities that promote quality patient care.
9. Advocate for patient rights and ensure that the best interests of the patient are always the priority.
10. Stay up to date on best practices and new developments in the field of social work.
