
Clinical Pharmacist Specialist
We are looking for a passionate, dedicated, and highly experienced Clinical Pharmacist Specialist to join our team at Children's Hospital Colorado. In this position, you will work closely with our medical teams to provide specialized clinical pharmacy services to pediatric patients. The ideal Clinical Pharmacist Specialist must have a Doctor of Pharmacy degree and be licensed to practice pharmacy in Colorado. A minimum of two years of clinical pharmacy experience in a pediatric setting, familiarity with medication safety and quality improvement principles, and experience in a healthcare team environment are also required. Responsibilities:
- Develop and implement clinical pharmacy services for pediatric patients.
- Consult with medical teams to ensure the safe and appropriate use of medications in pediatric patients.
- Monitor patient outcomes to assess the effectiveness of medications and make necessary changes.
- Educate patients, families, and medical staff on medication use and side effects.
- Participate in quality improvement initiatives and patient safety activities.
- Ensure medication orders are accurate and up to date.
- Maintain records of patient medication use, including dosage, duration, and side effects.
- Collaborate with other healthcare professionals to provide comprehensive patient care.
- Stay current on new developments in the field of pharmacy and pediatric care.
- Provide guidance and training to staff members and students.
