Children's Hospital Colorado is seeking an experienced and highly competent Clinical Pharmacist Specialist – Pharmacogenomics to join our dynamic team of healthcare professionals. Our ideal candidate will have a deep understanding of pharmacogenomics, extensive experience as a clinical pharmacist, and a commitment to providing the highest quality of patient care. The Clinical Pharmacist Specialist – Pharmacogenomics will be responsible for delivering the highest quality of clinical pharmacy services and caring for pediatric patients in an acute care setting. The successful candidate will be a motivated individual with a passion for pharmacogenomics, excellent communication skills, and an uncompromising dedication to patient safety.Required Qualifications: • Bachelor's of Science in Pharmacy• Minimum of 5 years of clinical pharmacy experience in an acute care setting• Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory Care Pharmacist (BCACP)• Advanced knowledge of pharmacogenomics• Knowledge of pediatric pharmacotherapeutics• Ability to work collaboratively in a team-based environment• Self-directed and motivated with a commitment to professional development
Responsibilities:
- Provide high-quality clinical pharmacy services for pediatric patients in an acute care setting.
- Utilize advanced knowledge of pharmacogenomics to ensure patient safety and optimal outcomes.
- Collaborate with team members to provide comprehensive patient care.
- Stay up to date with developments in pharmacogenomics and pediatric pharmacotherapeutics.
- Ensure compliance with all relevant laws and regulations.
- Maintain patient records and medical histories.
- Participate in professional development opportunities to stay up to date with the latest trends in pharmacogenomics.
- Participate in quality improvement initiatives to enhance patient care.
- Provide education and guidance to healthcare staff on pharmacogenomics and pediatric pharmacotherapeutics.
- Assess and evaluate patient response to medications and make appropriate changes to dosages and treatments when needed.
