  1. Develop and implement medication safety protocols and procedures for pediatric patients.
  2. Conduct regular audits and inspections to identify potential medication errors and areas for improvement.
  3. Collaborate with healthcare providers to develop individualized medication plans for pediatric patients.
  4. Stay up-to-date on the latest medication safety practices and guidelines in pediatrics.
  5. Provide education and training to healthcare staff on medication safety practices.
  6. Conduct medication reconciliation and review for pediatric patients.
  7. Investigate and report medication errors, near-misses, and adverse drug reactions.
  8. Identify and communicate potential risks and safety concerns related to medication use.
  9. Work closely with pharmacy staff to ensure safe and accurate medication dispensing and administration.
  10. Participate in multidisciplinary committees and teams to promote medication safety initiatives.
  11. Develop and maintain relationships with external organizations and agencies to stay informed about medication safety trends and best practices.
  12. Serve as a resource for patients, families, and caregivers on medication safety practices and concerns.
  13. Monitor and analyze medication-related data to identify trends and opportunities for improvement.
  14. Collaborate with IT and other departments to implement technology solutions for medication safety.
  15. Participate in quality improvement initiatives related to medication safety.
  16. Adhere to all regulatory requirements and maintain compliance with medication safety standards.
  17. Maintain accurate and detailed documentation of all medication safety activities.
  18. Communicate and collaborate effectively with all members of the healthcare team.
  19. Serve as a role model for medication safety and foster a culture of safety within the organization.
  20. Continuously evaluate and improve medication safety processes and procedures.

Job Qualifications
  • Bachelor's Degree In Pharmacy From An Accredited Institution

  • Current Pharmacist License In The State Of Colorado

  • Minimum Of 3 Years Experience Working As A Pharmacist In A Hospital Or Pediatric Setting

  • Certification In Medication Safety, Such As Through The Institute For Safe Medication Practices (Ismp)

  • Strong Knowledge Of Drug Interactions, Medication Error Prevention, And Adverse Drug Reactions In Pediatric Patients
