Community Based Social Worker

Responsibilities:

1. Provide care and support to patients and their families in the community
2. Develop and implement effective treatment plans for clients
3. Facilitate referrals to community resources and services
4. Provide consultation and training to other staff members
5. Monitor and evaluate progress of clients
6. Utilize a variety of therapeutic techniques to address the needs of clients
7. Maintain up-to-date client records
8. Produce timely and accurate reports and assessments
9. Participate in team and program meetings
10. Ensure compliance with relevant laws and regulations
