We are currently seeking a dedicated and compassionate Continuity Nurse to join our Inpatient Oncology team. In this role, you will play an integral part in providing continuous, comprehensive care for our pediatric oncology patients throughout their hospital stay. As a Continuity Nurse, you will have the opportunity to make a meaningful impact on the lives of children and their families during a challenging time. To be successful in this role, you must have a strong background in pediatric oncology and possess excellent communication and critical thinking skills.   1. Provide continuous, comprehensive care for pediatric oncology patients throughout their hospital stay.
  2. Collaborate with the interdisciplinary team to develop and implement individualized care plans for patients.
  3. Monitor and assess patient's physical and emotional well-being, and communicate any changes to the healthcare team.
  4. Administer medications and treatments as prescribed, and ensure patient safety at all times.
  5. Educate patients and their families about their medical condition, treatment plan, and self-care techniques.
  6. Assist in coordinating and scheduling appointments, procedures, and consultations for patients.
  7. Serve as a resource and support for patients and their families, addressing any questions or concerns they may have.
  8. Document patient care accurately and timely in electronic medical records.
  9. Participate in patient rounds and team meetings to discuss patient progress and plan of care.
  10. Stay up-to-date on current trends and advancements in pediatric oncology care.
  11. Adhere to hospital policies, procedures, and standards of care.
  12. Provide emotional support and comfort to patients and their families during a difficult time.
  13. Maintain a professional and compassionate demeanor at all times.
  14. Collaborate with other healthcare professionals and community resources to ensure continuity of care for patients after discharge.
  15. Attend trainings and educational opportunities to enhance knowledge and skills in pediatric oncology care.
  16. Adhere to infection control and safety protocols to ensure a safe and healthy environment for patients.
  17. Act as a patient advocate, ensuring their needs and wishes are respected and addressed.
  18. Maintain confidentiality of patient information in accordance with HIPAA regulations.
  19. Foster a positive and supportive work environment by communicating effectively and respectfully with colleagues.
  20. Uphold the mission and values of the Children's Hospital of Philadelphia in all interactions with patients, families, and colleagues.

Job Qualifications
  • Bachelor's Degree In Nursing: A Continuity Nurse In Inpatient Oncology At Children's Hospital Of Philadelphia Should Possess A Bachelor's Degree In Nursing From An Accredited Institution. This Education Provides A Strong Foundation In Nursing Principles, Patient Care, And Critical Thinking Skills.

  • Registered Nurse (Rn) License: The Candidate Should Hold A Current, Unrestricted Rn License In The State Of Pennsylvania. This License Ensures The Nurse Is Qualified To Practice In An Acute Care Setting And Is Knowledgeable About State-Specific Regulations.

  • Oncology Nursing Experience: The Ideal Candidate For This Position Should Have A Minimum Of 2 Years Of Experience In An Oncology Setting. This Experience Should Include Caring For Pediatric Patients With Various Types Of Cancer And Familiarity With Chemotherapy Treatments And Their Potential Side Effects.

  • Bls And Pals Certification: The Continuity Nurse Should Hold Current Basic Life Support (Bls) And Pediatric Advanced Life Support (Pals) Certifications. These Certifications Demonstrate The Nurse's Ability To Respond To Medical Emergencies And Provide Advanced Life Support To Pediatric Patients.

  • Excellent Communication And Organizational Skills: A Continuity Nurse In Inpatient Oncology Should Have Strong Communication Skills To Effectively Collaborate With The Interdisciplinary Team And Communicate With Patients And Their Families. They Should Also Possess Excellent Organizational Skills To Manage Patient Care And Ensure Continuity Of Care During Transitions Between Inpatient And Outpatient Settings.
