Social Work Service Coordinator

Children’s Hospital of Philadelphia is looking for an experienced Social Work Service Coordinator to join our team. This position is an exciting opportunity for someone who is passionate about making a difference in the lives of children and their families.The Social Work Service Coordinator will provide comprehensive and age-appropriate social work services including assessment, treatment planning, and intervention to patients and families. He/She will collaborate with other professionals and disciplines to ensure the delivery of quality care and services to meet the psycho-social needs of patients and families.The successful candidate must have a Master’s degree in Social Work and a minimum of two years of clinical experience providing direct social services to children and families in a medical setting. The ideal candidate will have excellent communication and interpersonal skills, be organized and detail-oriented, and have the ability to work independently. The Social Work Service Coordinator must possess a valid license to practice social work in the state of Pennsylvania.

Responsibilities:

1. Provide comprehensive and age-appropriate social work services, including assessment, treatment planning, and intervention, to patients and families.
2. Collaborate with other professionals and disciplines to ensure the delivery of quality care and services to meet the psycho-social needs of patients and families.
3. Maintain a valid license to practice social work in the state of Pennsylvania.
4. Develop and deliver educational programming to patients and families.
5. Maintain accurate and timely documentation of client progress, interventions, and services provided.
6. Facilitate referrals to community resources and provide support to patients and families throughout the referral process.
7. Participate in clinical meetings and case reviews.
8. Monitor patient progress and adjust services as needed.
9. Remain up-to-date on best practices related to social work and child welfare.
10. Establish and maintain effective communication with patients, families, and other professionals.
