
Social Worker (Healthy Weight Department)

- Provide individual and group counseling to children and adolescents struggling with weight management, utilizing evidence-based interventions and techniques.
- Conduct thorough assessments of patients' social and emotional needs and develop personalized treatment plans in collaboration with the multidisciplinary team.
- Monitor and track patients' progress and make adjustments to treatment plans as needed.
- Communicate and collaborate effectively with patients, their families, and the multidisciplinary team to ensure continuity of care.
- Advocate for the needs of patients and their families, and provide appropriate resources and referrals.
- Stay up-to-date on current research and best practices in the field of weight management, and incorporate this knowledge into treatment plans.
- Maintain accurate and thorough documentation of patient interactions and progress.
- Participate in team meetings and case conferences to discuss patient progress and treatment strategies.
- Provide education and support to patients and their families on healthy eating habits, physical activity, and overall wellness.
- Adhere to ethical standards and guidelines set forth by the National Association of Social Workers and the hospital.
- Maintain confidentiality and respect the privacy of patients and their families.
- Collaborate with community organizations and resources to provide additional support and services to patients and their families.
- Participate in ongoing professional development and training opportunities to enhance skills and knowledge in the field of social work.
- Contribute to the overall mission and goals of the Healthy Weight Department.
- Demonstrate compassion, empathy, and patience in working with children and families.
Bachelor's Or Master's Degree In Social Work From An Accredited Institution.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team.
At Least 2 Years Of Experience In A Healthcare Setting, Preferably In A Pediatric Or Family Medicine Setting.
Knowledge And Understanding Of Healthy Weight Management Principles And Strategies.
State Licensure As A Social Worker (Lcsw) Or Eligibility To Obtain Licensure Within 6 Months Of Hire.
