  1. Provide emotional support and counseling to children with Type 1 Diabetes and their families.
  2. Collaborate with medical team to develop comprehensive treatment plans for patients.
  3. Conduct assessments to identify social, emotional, and financial needs of patients and their families.
  4. Assist families in navigating the healthcare system and accessing necessary resources.
  5. Educate patients and families about the management of Type 1 Diabetes and how to cope with the challenges it presents.
  6. Advocate for patients and families to ensure their needs are met within the hospital and community.
  7. Coordinate with community organizations and agencies to provide additional support and resources for patients and families.
  8. Document patient progress and maintain accurate records.
  9. Participate in interdisciplinary team meetings and provide valuable input on patient care.
  10. Stay current on advancements and best practices in the treatment of Type 1 Diabetes to provide the most effective care.
  11. Maintain confidentiality and adhere to ethical standards of social work practice.
  12. Provide crisis intervention and support during difficult situations.
  13. Collaborate with other social workers and professionals within the hospital to provide a holistic approach to patient care.
  14. Act as a liaison between patients, families, and the hospital to ensure effective communication and understanding.
  15. Continuously assess and evaluate the effectiveness of interventions and make necessary adjustments to ensure optimal patient outcomes.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited University

  • Minimum Of 2 Years Experience Working In A Hospital Or Healthcare Setting

  • Knowledge Of Type 1 Diabetes And Its Impact On Children And Families

  • Experience In Providing Psychosocial Support And Counseling To Patients And Families

  • Strong Communication And Collaboration Skills To Work With Interdisciplinary Teams And Community Resources.
