Clinical Social Worker

Responsibilities: -Provide professional social work services to patients and families throughout the health system -Assess, diagnose, and treat patients with physical and mental health issues -Collaborate with other health professionals to ensure comprehensive patient care -Develop and implement individualized treatment plans for patients -Engage in ongoing clinical assessment and provide crisis intervention -Educate patients and families on available services and resources -Monitor patient progress and modify treatment plans as needed -Maintain accurate patient records and documentation -Comply with relevant laws, regulations, and ethical standards
