Job Description

In this role, you will play a crucial part in improving the overall health and well-being of our patients by implementing population health strategies and managing care coordination for our diverse patient population.   1. Develop and implement population health strategies to improve the overall health and well-being of our patients.
  2. Manage care coordination for a diverse patient population, ensuring that all patients receive high-quality and comprehensive healthcare services.
  3. Collaborate with various healthcare professionals, including physicians, nurses, and social workers, to develop individualized care plans for patients.
  4. Monitor and analyze population health data to identify areas for improvement and develop actionable plans to address these areas.
  5. Act as a liaison between patients and healthcare providers, ensuring that all communication and care plans are effectively communicated and coordinated.
  6. Establish and maintain relationships with community organizations and resources to support the health and well-being of patients.
  7. Provide education and support to patients and their families to promote self-management of chronic conditions and preventive care.
  8. Lead and participate in quality improvement initiatives to enhance the effectiveness and efficiency of population health interventions.
  9. Utilize electronic health record systems and other technology to track patient progress, identify gaps in care, and improve overall patient outcomes.
  10. Stay up-to-date with the latest developments and best practices in population health to continuously improve the care provided to our patients.

Job Qualifications
  • Bachelor's Degree In Healthcare Management, Public Health, Or A Related Field.

  • Minimum Of 3-5 Years Of Experience In Population Health Management Or Care Coordination.

  • Knowledge Of Healthcare Systems And Ability To Navigate And Collaborate With Various Stakeholders.

  • Strong Communication, Leadership, And Problem-Solving Skills.

  • Familiarity With Data Analysis And Ability To Use Data To Inform Decision Making And Improve Population Health Outcomes.
