  1. Develop and implement population health initiatives and programs to improve the overall health and well-being of the community.
  2. Conduct research and analyze data to identify healthcare disparities and develop strategies to address them.
  3. Collaborate with healthcare providers and community organizations to promote and support population health initiatives.
  4. Monitor and evaluate the effectiveness of population health programs and make recommendations for improvement.
  5. Stay current on industry trends and best practices in population health to inform program development.
  6. Communicate and present population health initiatives and outcomes to stakeholders, including senior leadership and community partners.
  7. Work closely with cross-functional teams to ensure alignment and integration of population health initiatives with other organizational goals and initiatives.
  8. Provide training and education to healthcare providers and staff on population health principles and practices.
  9. Collaborate with marketing and communications teams to develop and implement outreach and education campaigns to promote population health initiatives.
  10. Represent Duke University Health System at community events, conferences, and meetings related to population health.
  11. Maintain accurate and thorough documentation of population health programs, initiatives, and outcomes.
  12. Adhere to all relevant regulations, policies, and procedures related to population health and healthcare disparities.
  13. Mentor and support other team members in their roles related to population health.
  14. Continuously seek opportunities for improvement and innovation in population health practices.

Job Qualifications
  • Minimum Of A Master's Degree In Public Health, Epidemiology, Or Related Field.

  • At Least 3 Years Of Experience In Population Health Management, Preferably In A Healthcare Setting.

  • Knowledge And Understanding Of Healthcare Laws And Regulations, Specifically Related To Population Health And Accountable Care Organizations.

  • Strong Analytical And Data Management Skills, With Proficiency In Statistical Software And Electronic Health Records.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Diverse Stakeholders And Effectively Present Data And Findings To A Variety Of Audiences.
