
Transplant Coordinator - Registered Nurse

- Coordinate the transplantation process for patients, including pre-transplant evaluations, medical and psychological assessments, and post-transplant follow-up care.
- Serve as the primary point of contact for patients and their families, providing education and support throughout the transplantation journey.
- Collaborate with multidisciplinary teams, including surgeons, physicians, social workers, and dietitians, to develop individualized treatment plans for patients.
- Maintain accurate and up-to-date medical records and documentation related to the transplantation process.
- Monitor and assess patients' physical and emotional well-being before, during, and after transplantation, and provide appropriate interventions as needed.
- Educate patients and their families on transplant-related topics, including medications, self-care techniques, and potential complications.
- Act as a liaison between patients, healthcare providers, and insurance companies to ensure timely and efficient delivery of care.
- Participate in quality improvement initiatives and research projects related to organ transplantation.
- Stay current on advancements and developments in the field of organ transplantation, and incorporate new knowledge and best practices into patient care.
- Maintain strict adherence to ethical and legal standards, as well as organizational policies and procedures.
Registered Nurse License: The Candidate Must Hold An Active And Unrestricted Registered Nurse License In The State Of Georgia, Or Be Eligible For Licensure In The State.
Bachelor's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing Is Required For This Position. A Bachelor Of Science In Nursing (Bsn) Is Preferred.
Experience In Transplant Coordination: The Candidate Must Possess At Least 2-3 Years Of Experience In The Field Of Transplant Coordination, Preferably In A Hospital Or Healthcare Setting.
Strong Communication And Interpersonal Skills: The Candidate Must Have Excellent Communication Skills, Both Written And Verbal, And The Ability To Effectively Communicate With Patients, Families, And Healthcare Team Members.
Knowledge Of Transplant Regulations And Guidelines: The Candidate Must Have A Thorough Understanding Of Transplant Regulations And Guidelines, Including Unos And Cms Regulations, As Well As Experience Working With Transplant Coordinators, Surgeons, And Other Healthcare Professionals.
