  1. Coordinate the transplantation process for patients, including pre-transplant evaluations, medical and psychological assessments, and post-transplant follow-up care.
  2. Serve as the primary point of contact for patients and their families, providing education and support throughout the transplantation journey.
  3. Collaborate with multidisciplinary teams, including surgeons, physicians, social workers, and dietitians, to develop individualized treatment plans for patients.
  4. Maintain accurate and up-to-date medical records and documentation related to the transplantation process.
  5. Monitor and assess patients' physical and emotional well-being before, during, and after transplantation, and provide appropriate interventions as needed.
  6. Educate patients and their families on transplant-related topics, including medications, self-care techniques, and potential complications.
  7. Act as a liaison between patients, healthcare providers, and insurance companies to ensure timely and efficient delivery of care.
  8. Participate in quality improvement initiatives and research projects related to organ transplantation.
  9. Stay current on advancements and developments in the field of organ transplantation, and incorporate new knowledge and best practices into patient care.
  10. Maintain strict adherence to ethical and legal standards, as well as organizational policies and procedures.

Job Qualifications
  • Registered Nurse License: The Candidate Must Hold An Active And Unrestricted Registered Nurse License In The State Of Georgia, Or Be Eligible For Licensure In The State.

  • Bachelor's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing Is Required For This Position. A Bachelor Of Science In Nursing (Bsn) Is Preferred.

  • Experience In Transplant Coordination: The Candidate Must Possess At Least 2-3 Years Of Experience In The Field Of Transplant Coordination, Preferably In A Hospital Or Healthcare Setting.

  • Strong Communication And Interpersonal Skills: The Candidate Must Have Excellent Communication Skills, Both Written And Verbal, And The Ability To Effectively Communicate With Patients, Families, And Healthcare Team Members.

  • Knowledge Of Transplant Regulations And Guidelines: The Candidate Must Have A Thorough Understanding Of Transplant Regulations And Guidelines, Including Unos And Cms Regulations, As Well As Experience Working With Transplant Coordinators, Surgeons, And Other Healthcare Professionals.
