  1. Assist case managers in coordinating and managing patient care plans.
  2. Communicate with patients and their families to provide support and education on treatment plans and resources.
  3. Maintain accurate and up-to-date patient records and documentation.
  4. Collaborate with healthcare team members to ensure effective and efficient delivery of care.
  5. Schedule and coordinate appointments and services for patients.
  6. Monitor and track patient progress and report any changes or concerns to case managers.
  7. Assist in identifying and addressing any barriers to care for patients.
  8. Provide emotional support and guidance to patients and their families.
  9. Participate in team meetings and contribute to the development of care plans.
  10. Adhere to all relevant policies, procedures, and regulations.
  11. Maintain confidentiality and professionalism in all interactions.
  12. Continuously seek opportunities for personal and professional development.
  13. Advocate for patients' rights and needs.
  14. Collaborate with community resources to provide additional support and services for patients.
  15. Handle administrative tasks such as filing, data entry, and answering phone calls.
  16. Provide exceptional customer service to patients and their families.
  17. Adhere to all safety and infection control protocols.
  18. Assist with discharge planning and transition of care for patients.
  19. Perform other duties as assigned by case managers or supervisors.

Job Qualifications
