  1. Develop and implement strategies for integrating clinical and social services to improve the overall health and well-being of our patients.
  2. Oversee and manage the coordination of care between clinical and social service providers.
  3. Collaborate with other healthcare leaders to identify areas for improvement and implement initiatives to enhance patient outcomes.
  4. Develop and maintain relationships with community organizations and resources to expand the reach of our services.
  5. Monitor and analyze data to track the effectiveness of our integrated approach and make necessary adjustments.
  6. Ensure compliance with all relevant healthcare regulations and standards.
  7. Lead and mentor a team of clinical and social service professionals, providing guidance and support to help them achieve their goals.
  8. Develop and oversee budgets related to clinical and social services integration.
  9. Stay current on industry trends and advancements in healthcare integration to continuously improve our practices.
  10. Communicate and collaborate effectively with all levels of staff, patients, and external stakeholders.
  11. Foster a positive and inclusive work environment that values diversity and promotes teamwork.
  12. Participate in strategic planning and decision-making processes to advance the goals of the organization.
  13. Represent the organization at conferences, meetings, and other events to promote our integrated approach to healthcare.
  14. Continuously seek opportunities to improve processes and procedures for better patient outcomes.
  15. Ensure confidentiality and privacy of patient information and comply with all HIPAA regulations.

Job Qualifications
  • Advanced Degree In Healthcare Management Or A Related Field: A Director Of Clinical & Social Health Integration At Henry Ford Health System Should Possess A Master's Degree In Healthcare Management, Public Health, Or A Related Field. This Qualification Ensures That The Candidate Has The Necessary Knowledge And Skills To Lead And Manage The Integration Of Clinical And Social Health Services.

  • Extensive Experience In Healthcare Administration: The Ideal Candidate For This Role Should Have At Least 7-10 Years Of Experience In Healthcare Administration, With A Strong Background In Clinical And Social Health Services. This Experience Should Include Managing And Overseeing The Delivery Of Integrated Healthcare Services In A Complex Healthcare System.

  • Strong Knowledge Of Clinical & Social Health Integration: The Director Of Clinical & Social Health Integration At Henry Ford Health System Should Have A Deep Understanding Of The Principles And Practices Of Integrating Clinical And Social Health Services. This Includes Knowledge Of Evidence-Based Practices, Population Health Management, And Care Coordination.

  • Leadership And Strategic Thinking Skills: As A Key Leadership Position, The Director Of Clinical & Social Health Integration Should Possess Strong Leadership And Strategic Thinking Skills. This Includes The Ability To Collaborate Effectively With Other Healthcare Leaders, Develop And Implement Strategic Plans, And Drive Organizational Change.

  • Excellent Communication And Interpersonal Skills: The Director Of Clinical & Social Health Integration Will Be Responsible For Working With A Diverse Group Of Stakeholders, Including Healthcare Providers, Community Organizations, And Government Agencies. Therefore, Excellent Communication And Interpersonal Skills Are Essential For Building And Maintaining Relationships, Negotiating Partnerships, And Advocating For The Integration Of Clinical And Social Health Services.
