- Conduct thorough patient evaluations and assessments to determine eligibility for transplantation.
- Collaborate with transplant physicians and other healthcare team members to develop and implement individualized treatment plans.
- Perform and interpret diagnostic tests and procedures to monitor patient progress and identify potential complications.
- Administer medications and treatments as prescribed by physicians.
- Educate patients and their families on the transplant process, including pre- and post-operative care.
- Monitor and manage patients' post-operative recovery and provide follow-up care.
- Maintain accurate and timely documentation of patient care and treatment plans.
- Communicate effectively with patients, families, and other healthcare professionals to provide comprehensive and coordinated care.
- Stay current on the latest advancements and developments in transplantation medicine.
- Adhere to all hospital policies and procedures, as well as state and federal regulations.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Provide emotional support and counseling to patients and their families throughout the transplant process.
- Collaborate with other healthcare professionals to coordinate care for patients with complex medical needs.
- Serve as a resource and mentor for other healthcare team members.
- Uphold the values and mission of Henry Ford Health System in all interactions with patients, families, and colleagues.
Master's Degree In Physician Assistant Studies Or Nurse Practitioner Program.
Current Certification And Licensure As A Physician Assistant Or Nurse Practitioner In The State Of Michigan.
Minimum Of 2-3 Years Of Experience In A Transplant Setting, With A Focus On Pre- And Post-Transplant Care.
Strong Knowledge And Understanding Of Transplant Procedures, Immunosuppressive Medications, And Post-Transplant Complications.
Excellent Communication And Teamwork Skills, With The Ability To Collaborate With Transplant Surgeons, Physicians, And Other Healthcare Professionals.
