  1. Develop and maintain relationships with community organizations and resources to identify potential patients in need of home health services.
  2. Conduct thorough assessments of patients' needs and develop individualized care plans based on their specific conditions and circumstances.
  3. Collaborate with interdisciplinary care team to coordinate and provide comprehensive care to patients in their homes.
  4. Educate patients and their families on disease management, medication management, and healthy lifestyle choices.
  5. Monitor and evaluate patients' progress and adjust care plans as needed to ensure optimal outcomes.
  6. Communicate effectively with patients, families, and caregivers to provide emotional support and address any concerns or questions.
  7. Maintain accurate and thorough documentation of patient care, including progress notes, assessments, and care plans.
  8. Ensure compliance with all regulatory and accreditation standards related to home health care.
  9. Participate in quality improvement initiatives to enhance patient care and outcomes.
  10. Serve as a liaison between patients, their families, and community resources to help meet their needs and improve their overall well-being.

Job Qualifications
  • Current And Valid Rn License In The State Of Michigan.

  • Bachelor's Degree In Nursing From An Accredited Institution.

  • Minimum Of 2 Years Of Experience In Community Health/Home Health Nursing.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Interdisciplinary Healthcare Teams.

  • Knowledge Of Medicare And Medicaid Regulations And Guidelines For Home Health Services.
