Job Description

HOAG is seeking a compassionate and dedicated Registered Nurse to join our Admissions team. As an Admissions RN, you will play a crucial role in providing exceptional patient care by assessing and coordinating all aspects of the patient's admission process.   1. Conduct thorough patient assessments to determine appropriate level of care and admission process.
  2. Coordinate with patients, families, and healthcare team to ensure a seamless and efficient admission process.
  3. Provide compassionate and personalized care to patients during the admission process.
  4. Educate patients and families on the admission process, including any necessary procedures and treatments.
  5. Collaborate with physicians and other healthcare professionals to develop an individualized care plan for each patient.
  6. Monitor and track patient progress during the admission process, making necessary adjustments to the care plan as needed.
  7. Communicate effectively with patients, families, and healthcare team to ensure all parties are informed and involved in the admission process.
  8. Maintain accurate and detailed documentation of all patient interactions and progress.
  9. Adhere to all hospital policies, procedures, and standards of care to ensure the highest quality of patient care.
  10. Continuously strive to improve patient outcomes and satisfaction through ongoing education and training.
  11. Serve as a patient advocate, ensuring that all patient needs and concerns are addressed and resolved.
  12. Collaborate with insurance providers to verify coverage and obtain necessary authorizations for admission.
  13. Maintain a safe and clean environment for patients and staff, following infection control and safety protocols.
  14. Participate in departmental and interdisciplinary meetings to discuss patient care and address any issues or concerns.
  15. Serve as a mentor and resource for other team members, promoting a positive and supportive work environment.

Job Qualifications
  • Current Rn License In The State Of California.

  • Minimum Of 2 Years Of Experience In A Hospital Setting, Preferably In Admissions Or A Related Field.

  • Strong Communication And Interpersonal Skills To Effectively Interact With Patients, Families, And Healthcare Team Members.

  • Knowledge Of Medical Terminology, Insurance Requirements, And Admission Procedures.

  • Ability To Work Collaboratively With Other Healthcare Professionals And Handle Multiple Tasks In A Fast-Paced Environment.
