Job Description

The Massachusetts General Hospital is seeking a qualified Palliative Care Case Manager to join our team of dedicated professionals. Working closely with our physicians, nurses, and other team members, this position will be a vital part of providing excellent patient-centered care to our palliative care patients. The Palliative Case Manager will coordinate the patient’s care across all disciplines, provide support to the patient and their families, and ensure the delivery of quality care.To be considered for this role, applicants must have a Bachelor’s degree in Nursing along with a minimum of five years of clinical experience in nursing and a minimum of three years in palliative care. Additionally, experience with Electronic Health Record (EHR) systems and palliative care certification is preferred.We are looking for an individual who is passionate about providing excellent palliative care and who is able to work both independently and collaboratively as part of a team. The ideal candidate must be flexible, self-motivated, and possess excellent communication and interpersonal skills. Above all, the successful candidate must demonstrate a commitment to providing compassionate care to those in our care.

Responsibilities:
