Molina Healthcare is seeking a dedicated and compassionate IRIS Consultant - TMG to join our team. As an IRIS Consultant, you will play a critical role in providing person-centered support and services to our members. This position requires a highly organized and empathetic individual with excellent communication skills and a deep understanding of the unique needs of individuals with disabilities.   1. Conduct initial and ongoing assessments of members' needs and goals to develop comprehensive service plans.
  2. Collaborate with members, their families, and other healthcare professionals to identify and address barriers to care.
  3. Provide person-centered support and guidance to members to help them achieve their desired outcomes.
  4. Monitor and track member progress and make necessary adjustments to service plans as needed.
  5. Advocate for members' rights and ensure they are receiving appropriate and timely services.
  6. Maintain accurate and up-to-date documentation of all member interactions and services provided.
  7. Stay current on all state and federal regulations and policies related to intellectual and developmental disabilities.
  8. Educate members and their families on available resources and services to enhance their quality of life.
  9. Coordinate and collaborate with community organizations and agencies to connect members with additional support and services.
  10. Participate in regular team meetings and trainings to ensure best practices and quality of care.
  11. Develop and maintain positive relationships with members and their families to promote trust and open communication.
  12. Serve as a liaison between members, their families, and healthcare providers to ensure coordinated and comprehensive care.
  13. Continuously evaluate and improve processes to enhance member satisfaction and outcomes.
  14. Participate in outreach and education events to raise awareness and promote the IRIS program.
  15. Adhere to all ethical and confidentiality standards in accordance with HIPAA regulations.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or Related Field.

  • Minimum Of 2 Years Of Experience In Case Management Or Related Field.

  • Knowledge Of And Experience With Iris (Include, Respect, I Self-Direct) Program And Principles.

  • Strong Communication And Interpersonal Skills, With The Ability To Build Rapport And Maintain Relationships With Clients And Stakeholders.

  • Ability To Work Independently, Manage A Caseload, And Make Sound Decisions In A Fast-Paced Environment.
