- Dispense medications accurately and efficiently to patients in the Outpatient Clinical Services department.
- Conduct thorough medication reviews and ensure proper dosing and administration instructions are provided to patients.
- Collaborate with other healthcare professionals to develop and implement safe and effective medication therapy plans for patients.
- Monitor and assess patient response to medication therapy and make necessary adjustments to ensure optimal outcomes.
- Ensure compliance with all state and federal regulations and maintain accurate records of all medications dispensed.
- Educate patients on proper medication use, potential side effects, and drug interactions.
- Provide excellent customer service and address any patient concerns or questions.
- Participate in medication order verification and drug utilization review to ensure appropriate use of medications.
- Stay abreast of new medications, therapies, and treatment protocols through continuing education and professional development.
- Collaborate with the pharmacy team to maintain a safe and organized medication inventory.
- Assist in the training and supervision of pharmacy technicians and support staff.
- Adhere to all MUSC Health policies and procedures and promote a culture of patient-centered care.
- Maintain confidentiality of patient information and adhere to HIPAA regulations.
- Participate in quality improvement initiatives and contribute to the overall success of the Outpatient Clinical Services department.
- Foster a positive and collaborative work environment and serve as a role model for excellence in patient care.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy
Active And Unrestricted Pharmacist License In The State Of South Carolina
Minimum Of 2 Years Of Experience In A Clinical Pharmacy Setting, Preferably In An Outpatient Or Ambulatory Care Setting
Board Certification In A Relevant Specialty (E.g. Ambulatory Care, Pharmacotherapy, Etc.)
Strong Clinical Knowledge And Expertise In Medication Therapy Management, Drug Interactions, And Patient Counseling.
