  1. Provide comprehensive social work services to adolescent patients and their families, including psychosocial assessments, counseling, and resource referrals.
  2. Collaborate with multidisciplinary team members to develop and implement individualized treatment plans for adolescent patients.
  3. Assist in coordinating and facilitating communication between patients, families, and healthcare providers to ensure the best possible outcomes.
  4. Conduct ongoing assessments of patient and family needs and make appropriate referrals to community resources as needed.
  5. Advocate for the rights and needs of adolescent patients, including addressing any barriers to care they may face.
  6. Maintain accurate and timely documentation of patient interactions, treatment plans, and progress notes.
  7. Provide crisis intervention and support to patients and families as needed.
  8. Stay up-to-date on current trends and research in adolescent healthcare and social work to inform and improve practice.
  9. Participate in team meetings, trainings, and other professional development opportunities to enhance skills and knowledge.
  10. Represent MUSC Health in a professional and compassionate manner at all times, upholding the organization's values and mission.
  11. Collaborate with community partners and organizations to enhance support and resources for adolescent patients.
  12. Serve as a mentor and resource for other social work staff, students, and interns within the adolescent medicine department.
  13. Adhere to all ethical and legal standards set forth by the National Association of Social Workers and MUSC Health.
  14. Take an active role in quality improvement initiatives and contribute to the ongoing development and improvement of adolescent healthcare services at MUSC Health.
  15. Maintain confidentiality and respect the privacy of patients and families at all times.

Job Qualifications
  • Master's Degree In Social Work (Msw) From An Accredited Institution.

  • Licensure As A Licensed Clinical Social Worker (Lcsw) In The State Of South Carolina.

  • Minimum Of 2 Years Of Experience Working With Adolescents In A Healthcare Setting.

  • Knowledge And Understanding Of Adolescent Development, Mental Health Issues, And Chronic Medical Conditions.

  • Strong Communication And Interpersonal Skills, As Well As The Ability To Work Collaboratively With A Multidisciplinary Team.
