
Violence Prevention & Intervention Referral Coordinator

- Coordinate referrals for individuals seeking support and resources related to violence prevention and intervention.
- Work closely with a multidisciplinary team to ensure that patients and community members receive appropriate services and support.
- Conduct thorough assessments to determine individuals' needs and connect them with appropriate resources.
- Maintain accurate and up-to-date records of referrals and services provided.
- Collaborate with community organizations and agencies to expand resources and support for violence prevention and intervention.
- Educate patients and community members about violence prevention and intervention strategies.
- Monitor and evaluate the effectiveness of referral services and make recommendations for improvement.
- Stay current on best practices and evidence-based approaches in violence prevention and intervention.
- Develop and maintain relationships with key stakeholders in the community.
- Participate in trainings and professional development opportunities to enhance knowledge and skills in violence prevention and intervention.
- Serve as an advocate for social justice and community advocacy in all aspects of the role.
- Communicate effectively with patients, community members, and team members to ensure a coordinated and efficient referral process.
- Adhere to all ethical and legal standards related to confidentiality and patient care.
- Maintain a positive and professional demeanor at all times, representing MUSC Health and our mission to create a safer and more peaceful community.
- Other duties as assigned by the supervisor.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Minimum Of 2 Years Experience In A Violence Prevention Or Intervention Role.
Knowledge Of Local And National Resources For Victims Of Violence.
Strong Communication Skills And Ability To Work With Diverse Populations.
Experience With Case Management And Referral Coordination.
