Job Description

We are currently seeking a passionate and skilled Homecare Physical Therapist to join our team and help our patients improve their quality of life. As a Homecare Physical Therapist, you will have the opportunity to work one-on-one with patients in their homes, developing personalized treatment plans and providing hands-on therapy to help them regain or maintain their physical independence. If you have a strong background in physical therapy and a desire to make a positive impact on the lives of others, we would love to hear from you.

  1. Develop personalized treatment plans for patients based on their specific needs and goals.
  2. Conduct thorough assessments of patients to determine their physical therapy needs.
  3. Provide hands-on therapy to patients in their homes.
  4. Monitor and track patient progress, adjusting treatment plans as needed.
  5. Educate patients and their families on proper techniques and exercises to improve their physical wellbeing.
  6. Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for patients.
  7. Maintain accurate and timely documentation of patient progress and treatment plans.
  8. Adhere to all ethical and legal standards in the practice of physical therapy.
  9. Keep up-to-date with advancements and best practices in the field of physical therapy.
  10. Provide compassionate and attentive care to patients and their families.
  11. Communicate effectively with patients, families, and colleagues.
  12. Adhere to all safety protocols and guidelines to ensure a safe working environment for both patients and staff.
  13. Maintain confidentiality of patient information.
  14. Participate in team meetings and contribute to the development of best practices and protocols.
  15. Represent the organization in a positive and professional manner at all times.

Job Qualifications
  • Bachelor's Or Higher Degree In Physical Therapy From An Accredited Institution.

  • Valid State License To Practice As A Physical Therapist.

  • Minimum Of 2 Years Of Experience In Homecare Physical Therapy.

  • Strong Knowledge Of Various Physical Therapy Techniques And Modalities.

  • Excellent Communication And Time Management Skills, With The Ability To Work Independently In A Homecare Setting.
