- Develop and implement educational programs and materials for patients with diabetes, including individual and group sessions.
- Conduct assessments to determine patients' educational needs and develop personalized care plans.
- Provide ongoing counseling and support to patients with diabetes, addressing their physical, emotional, and social needs.
- Collaborate with other healthcare professionals to ensure comprehensive care for patients with diabetes.
- Stay up-to-date on the latest research and developments in diabetes care and incorporate them into educational programs.
- Monitor and evaluate the effectiveness of educational programs and make necessary adjustments for continuous improvement.
- Maintain accurate and timely documentation of patient interactions and progress.
- Educate patients on self-management techniques, including nutrition, exercise, blood glucose monitoring, and medication management.
- Promote patient empowerment and self-advocacy through education and support.
- Serve as a resource and advocate for patients with diabetes within the healthcare system.
- Participate in community outreach programs to raise awareness and educate the public about diabetes prevention and management.
- Continuously strive to improve patient outcomes and satisfaction.
- Adhere to all professional and ethical standards and guidelines.
- Collaborate with the healthcare team to identify and address any barriers to effective diabetes management.
- Act as a mentor and resource for other healthcare professionals on diabetes education and management.
Bachelor's Degree In Nursing, Nutrition, Or A Related Field.
Certified Diabetes Educator (Cde) Certification From The National Certification Board For Diabetes Educators (Ncbde).
Minimum Of 2 Years Of Experience In Diabetes Education And Management.
Strong Knowledge And Understanding Of Diabetes Management, Including Medication, Nutrition, And Lifestyle Modifications.
Excellent Communication And Teaching Skills, With The Ability To Educate Patients And Their Families On Diabetes Self-Management Techniques.
