Job Description

Novant Health is looking for an experienced Registered Nurse to join our dynamic team as a Diabetes Educator. This position will be critical in providing education and support to patients diagnosed with diabetes. We are looking for an individual who is passionate about helping people manage their diabetes and is committed to providing excellent patient care.The successful candidate for this role should have a Bachelor's Degree in Nursing and experience in providing diabetes education and support. Preference will be given to candidates who are certified by the National Certification Board for Diabetes Educators. The ideal candidate will be a team player with strong communication and interpersonal skills, and a compassionate, patient-centered approach to care.

Responsibilities:

  1. Provide education and support to patients diagnosed with diabetes.
  2. Develop and implement patient education and support plans based on individual patient needs.
  3. Monitor patient progress and adjust plans as needed.
  4. Collaborate with other healthcare professionals to ensure best practices are followed and patient outcomes are optimal.
  5. Develop and maintain relationships with patients and their families to ensure they are aware of available resources and services.
  6. Educate patients and family members about diabetes management, nutrition, exercise and lifestyle changes.
  7. Maintain current knowledge of diabetes management and treatments.
  8. Participate in continuing education activities and other professional development opportunities.
  9. Assist in developing and implementing standards of care for diabetes patients.
  10. Utilize appropriate documentation and coding practices.
