
Hospice & Palliative Care Advanced Practice Provider
Novant Health is seeking an experienced Hospice and Palliative Care Advanced Practice Provider to join our team of dedicated and compassionate providers. The successful candidate will bring a passion for providing exceptional care, make a positive impact on the patient experience, and work collaboratively with the rest of the team.The Hospice & Palliative Care Advanced Practice Provider must be a certified Nurse Practitioner or Physician Assistant with the appropriate licensure. They must also have experience in hospice and palliative care, as well as excellent communication and interpersonal skills. The ideal candidate will be committed to providing patient-centered, high-quality care and will possess a strong desire to make a difference in the lives of our patients.
Responsibilities:
- Provide hospice and palliative care services to patients in accordance with Novant Health standards.
- Demonstrate a commitment to providing high-quality, patient-centered care.
- Ensure compliance with all relevant regulations and laws.
- Develop and implement care plans that meet the individualized needs of each patient.
- Monitor patient’s progress and modify plans of care as necessary.
- Collaborate with other members of the healthcare team to ensure the best possible outcomes for patients.
- Provide emotional support to patients and their families.
- Educate patients and families about treatments and therapies.
- Document patient care in accordance with Novant Health standards.
- Maintain knowledge of current trends and developments in hospice and palliative care.
Board Certified/Eligible Advanced Practice Nurse Provider Or Physician Assistant
Master's Degree In Nursing Or Physician Assistant Program
Current Licensure In Appropriate State Or Compact State
Dea Licensure
Current Certification In Basic Life Support
Advanced Cardiac Life Support
And/Or Pediatric Advanced Life Support
Certification Or Eligibility In Hospice And Palliative Care
Knowledge And Experience In The Care Of The Terminally Ill Patient
Knowledge Of Professional Standards Of Care
