
Telephonic Nurse Case Manager RN
We are looking for a highly motivated Telephonic Nurse Case Manager RN with a passion for providing exceptional patient care to join our team at Optum. We are looking for an individual with the commitment and drive to help us meet our goals of providing the best care to our patients. The ideal candidate for this position will have excellent interpersonal communication and organizational skills, as well as a proven track record of managing complex patient care plans. This position requires an RN with a current license and at least 2 years of experience in nursing and/or case management. A successful candidate must have a successful track record of working with physicians, insurance companies, and other healthcare providers and display strong problem-solving skills. Responsibilities:
- Provide exceptional telephonic patient care and case management services.
- Develop and maintain relationships with physicians, insurance companies, and other healthcare providers.
- Communicate with patients to educate them on their care plans and answer questions.
- Monitor patient progress and adjust care plans as needed.
- Create detailed patient care plans and document all care provided.
- Collaborate with other healthcare providers to coordinate patient care.
- Ensure all patient records are accurate and up-to-date.
- Follow all applicable laws and regulations regarding patient care.
- Update medical charts and provide accurate reports as needed.
- Act as a liaison between patients, families, and other healthcare providers.
