As a Nurse Practitioner in our Geriatrics department, you will play a crucial role in providing comprehensive care to our elderly patients and promoting their overall health and independence.   1. Provide comprehensive care to elderly patients in the Geriatrics department.
  2. Conduct thorough assessments of patients' physical, mental, and emotional health.
  3. Develop personalized care plans in collaboration with the interdisciplinary team.
  4. Monitor patients' progress and make necessary adjustments to care plans.
  5. Administer medications and treatments as prescribed by physicians.
  6. Educate patients and their families on disease management, healthy lifestyle choices, and medication adherence.
  7. Collaborate with other healthcare professionals to ensure coordinated and holistic care.
  8. Document patient care accurately and in a timely manner.
  9. Stay up-to-date with current trends and advancements in geriatric care.
  10. Advocate for the needs and rights of elderly patients.
  11. Maintain confidentiality and adhere to ethical and legal standards of practice.
  12. Act as a resource and mentor for other healthcare team members.
  13. Participate in quality improvement initiatives and contribute to the development of best practices.
  14. Maintain a safe and clean environment for patients and staff.
  15. Demonstrate compassion, empathy, and respect towards patients and their families.
  16. Adhere to all policies, procedures, and regulations of Providence St. Joseph Health.

Job Qualifications
  • Master's Degree In Nursing With A Focus On Geriatrics: The Ideal Candidate For This Position Should Possess A Minimum Of A Master's Degree In Nursing With A Specialization In Geriatrics. This Will Ensure That The Individual Has The Necessary Knowledge And Skills To Provide Specialized Care To Elderly Patients.

  • Active Nurse Practitioner (Np) License: A Valid And Active Np License Is A Must-Have For This Role. This License Should Be In Good Standing And Should Be Issued By The State In Which The Candidate Will Be Practicing.

  • Certification In Gerontology Or Geriatric Nursing: Candidates With Additional Certifications In Gerontology Or Geriatric Nursing Will Be Given Preference. This Demonstrates A Commitment To Ongoing Education And A Deeper Understanding Of The Unique Needs Of Elderly Patients.

  • Minimum Of 2 Years Of Experience In Geriatrics: To Be Considered For This Role, Candidates Should Have A Minimum Of 2 Years Of Experience Working As A Nurse Practitioner In A Geriatrics Setting. This Experience Will Ensure That The Individual Is Familiar With The Challenges And Complexities Of Caring For Elderly Patients.

  • Strong Communication And Interpersonal Skills: As A Nurse Practitioner In Geriatrics, The Candidate Will Be Working Closely With Patients, Their Families, And Other Healthcare Professionals. Therefore, Strong Communication And Interpersonal Skills Are Essential For Building Trust, Establishing Rapport, And Providing Quality Care To Patients.
