  1. Serve as the first point of contact for patients, providing a warm and welcoming experience at all times.
  2. Guide and support patients through their healthcare journey, ensuring they receive the necessary care and resources.
  3. Conduct initial assessments and screenings to determine the patient's needs and concerns.
  4. Collaborate with healthcare providers to develop personalized care plans for patients.
  5. Provide education and information to patients about their medical conditions, treatment options, and available resources.
  6. Assist patients in navigating the healthcare system, including scheduling appointments, coordinating referrals, and obtaining insurance coverage.
  7. Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with privacy regulations.
  8. Act as an advocate for patients, addressing any barriers to care and ensuring their needs are met.
  9. Continuously evaluate and improve patient navigation processes to enhance the patient experience.
  10. Collaborate with other members of the healthcare team to ensure seamless and coordinated care for patients.
  11. Stay informed on current healthcare policies, trends, and resources to better assist patients.
  12. Uphold the mission, vision, and values of Providence St. Joseph Health in all interactions with patients and colleagues.
  13. Participate in team meetings, trainings, and professional development opportunities to enhance skills and knowledge.
  14. Adhere to all legal, ethical, and regulatory standards in the performance of job duties.

  16. Maintain a positive and professional attitude, even in challenging situations.
  17. Provide feedback and suggestions for improvement to enhance the patient experience.

  19. Ensure timely and accurate documentation of all patient interactions and activities.
  20. Perform any other duties as assigned by the supervisor to support the overall functioning of the patient navigation program.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Social Work, Or A Related Field.

  • Minimum Of 2 Years Experience In A Healthcare Setting, Preferably In A Patient Navigation Or Care Coordination Role.

  • Strong Knowledge Of Healthcare Systems, Insurance, And Community Resources.

  • Excellent Communication And Interpersonal Skills, With The Ability To Effectively Advocate For Patients And Collaborate With Healthcare Teams.

  • Experience Working With Diverse Populations And Understanding Of Cultural Competency.
