- Coordinate and manage patient care services to ensure timely and high-quality care for all patients.
- Collaborate with healthcare providers to develop and implement individualized care plans for patients.
- Serve as a point of contact for patients and their families, addressing any questions or concerns and providing support and guidance.
- Maintain accurate and up-to-date records of patient care and treatment plans.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Educate patients and their families on their conditions, treatment plans, and available resources.
- Work closely with other healthcare professionals to ensure seamless coordination of care.
- Assist in the scheduling of appointments and procedures for patients.
- Provide guidance and support to junior patient care coordinators.
- Stay up-to-date on industry trends and best practices in patient care to continuously improve and enhance services.
- Adhere to all relevant healthcare regulations and protocols.
- Maintain confidentiality and professionalism in all interactions with patients and their families.
- Attend meetings and training sessions as required.
- Serve as a patient advocate, ensuring that their needs and preferences are heard and respected.
- Handle any administrative tasks related to patient care coordination, such as insurance verification and billing.
Minimum Of 5 Years Experience In A Healthcare Setting, Preferably In A Patient Care Coordination Role.
Bachelor's Degree In Healthcare Administration, Nursing, Or Related Field.
Strong Knowledge Of Medical Terminology, Insurance Processes, And Patient Care Coordination Best Practices.
Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Multidisciplinary Teams.
Experience With Electronic Medical Records Systems And Proficiency In Microsoft Office Suite.
