Social Worker - Primary Care Behavioral Health

1. Provide integrated behavioral health services to patients in a primary care setting.
2. Collaborate with healthcare team members to assess, diagnose, and develop treatment plans for patients.
3. Conduct psychosocial assessments and provide appropriate interventions.
4. Offer counseling and support to patients and their families.
5. Educate patients on coping skills, stress management, and other behavioral health techniques.
6. Facilitate referrals to community resources and coordinate care with external providers.
7. Maintain accurate and timely documentation of patient assessments, treatment plans, and progress.
8. Monitor and evaluate patient progress and adjust treatment plans as needed.
9. Advocate for patients and ensure their needs are met in a compassionate and culturally sensitive manner.
10. Participate in team meetings and case conferences to discuss patient care and develop strategies for improving services.
11. Stay up-to-date on best practices and evidence-based treatments in social work and behavioral health.
12. Adhere to all ethical and legal standards in the provision of services.
13. Maintain confidentiality and respect patients' rights to privacy.
14. Collaborate with community organizations and agencies to address social determinants of health.
