
Social Worker - Primary Care Behavioral Health

As a Social Worker, you will play an integral role in providing integrated behavioral health services to patients in our primary care setting. - Provide integrated behavioral health services to patients in a primary care setting.
- Collaborate with healthcare team members to assess, diagnose, and develop treatment plans for patients.
- Conduct psychosocial assessments and provide appropriate interventions.
- Offer counseling and support to patients and their families.
- Educate patients on coping skills, stress management, and other behavioral health techniques.
- Facilitate referrals to community resources and coordinate care with external providers.
- Maintain accurate and timely documentation of patient assessments, treatment plans, and progress.
- Monitor and evaluate patient progress and adjust treatment plans as needed.
- Advocate for patients and ensure their needs are met in a compassionate and culturally sensitive manner.
- Participate in team meetings and case conferences to discuss patient care and develop strategies for improving services.
- Stay up-to-date on best practices and evidence-based treatments in social work and behavioral health.
- Adhere to all ethical and legal standards in the provision of services.
- Maintain confidentiality and respect patients' rights to privacy.
- Collaborate with community organizations and agencies to address social determinants of health.
- Demonstrate a commitment to the mission and values of Providence St. Joseph Health.
Master's Degree In Social Work From An Accredited University.
Current State Licensure As A Licensed Clinical Social Worker (Lcsw).
Minimum Of 2-3 Years Of Experience Providing Direct Clinical Counseling In A Primary Care Or Behavioral Health Setting.
Knowledge And Understanding Of Evidence-Based Practices For Behavioral Health Treatment, Including Cognitive Behavioral Therapy And Motivational Interviewing.
Ability To Work Collaboratively With Interdisciplinary Teams And Effectively Communicate With Patients From Diverse Backgrounds.
