Job Description

Providence St. Patrick Hospital is looking for a motivated and caring Case Management Associate to join our team. In this role, you will be responsible for coordinating and managing the care of patients with acute and chronic health conditions. You must be organized and have excellent communication skills to ensure successful case management and collaboration with physicians, nurses and other healthcare professionals. To be successful in this role, you must have a Bachelor’s degree in healthcare or social services, or a related field. You should also have at least one year of experience in a healthcare or social services setting. Previous case management experience is preferred.As a Case Management Associate, you must be compassionate and have strong critical thinking skills. You should be committed to providing excellent patient care and must be flexible to adjust to different situations. Responsibilities:

  1. Coordinate and manage the care of patients with acute and chronic health conditions.
  2. Maintain excellent communication with physicians, nurses, and other healthcare professionals to ensure successful case management.
  3. Provide compassionate care and critical thinking to ensure successful patient outcomes.
  4. Maintain strong organizational and time management skills to manage multiple cases at any given time.
  5. Monitor and evaluate patient progress and adjust care plans as needed.
  6. Develop and implement patient care plans in accordance with established guidelines.
  7. Educate patients and families on healthcare and treatment plans.
  8. Document patient care plans and progress in accordance with established protocols.
  9. Participate in team meetings and other related activities.
  10. Assist in the development and implementation of case management initiatives and policies.
